Nomination Guidelines
The David Lawrence Community Service Awards nomination process will begin again in 2013
Do you know of an individual or group whose work exemplifies Kaiser Permanente's dedication to improving the health of our communities? If so, please consider nominating them for a David Lawrence Community Service Award. For information on previous winners, click here.
The David Lawrence Community Service Awards are named in honor of Dr. David Lawrence, former CEO of Kaiser Permanente and lifelong advocate and champion of improving community health. The Awards recognize individuals and groups across the Kaiser Permanente regions who champion outstanding community benefit activities and initiatives demonstrating extraordinary efforts to improve the health of our communities. These efforts may be completely voluntary, or they may be related to a person's job or a group's function within Kaiser Permanente. For the second year, international volunteer service will be eligible for the award.
The accomplishments of awardees are profiled in electronic and print media and in Kaiser Permanente publications throughout the year. Each winner is recognized with a $10,000 donation to designate to a nonprofit organization.
Nominations for individual and group Awards may be submitted by any physician or employee of Kaiser Permanente Health Plan or Hospitals or any Permanente Medical Group. Nominees must be full or part-time permanent employees of Kaiser Permanente during the year in which they are nominated. Self-nominations are also welcome. As not all exemplary efforts can receive the Award in a given year, individuals and groups who are not selected can be renominated the following year.
Finalists and their nominators may be interviewed as part of the due diligence process. Winners will be chosen by a formal selection committee of the Kaiser Foundation Health Plan/Hospitals Board of Directors. Thank you for your interest in this special Award.
Individual nominees
- Exemplary efforts
- A significant history of sustained contributions to the overall health of a community or population which extends outside of Kaiser Permanente membership.
- A measurable impact on the health or well-being of a community or population.
- A commitment to important health and social issues, including, but not limited to:
- Social determinants of health: social and environmental factors which are root causes of disease and health disparities
- Culturally competent care
- Care and coverage for vulnerable populations
- Collaboration with a community organization or NGO.
- Understanding of racial and ethnic disparities in health care
Group nominees
The group must demonstrate:- Voluntary, replicable or collaborative efforts
- A significant history of sustained contributions to the overall health of a community or population which extends outside of Kaiser Permanente membership.
- A measurable impact on the health or well-being of a community or population.
- A commitment to important health and social issues, including, but not limited to:
- Social determinants of health: social and environmental factors which are root causes of disease and health disparities
- Culturally competent care
- Care and coverage for vulnerable populations
- Collaboration with a community organization or NGO.
- Understanding of racial and ethnic disparities in health care
- Contribution to the facility's Community Benefit Plan (where appropriate).
