      A healthy future starts today

       

      Health affects everything from physical and emotional well-being to performance in school. By enrolling your children in Kaiser Permanente’s Child Health Program (CHP), you can ensure that your children receive the health care they need to thrive. Depending on your family size and income, your monthly payment amount will be $0, $10 or $20 per child, for up to three children. Additional children are covered at no extra cost.

      CHP provides a subsidy to lower your monthly payment amounts for enrollment in the Kaiser Permanente Platinum─90 HMO plan which includes dental coverage through Delta Dental of California.

      CHP also includes medical financial assistance that eliminates out-of-pocket costs for most covered services provided at a Kaiser Permanente medical office or hospital. If services are not provided at a Kaiser Permanente medical office or hospital you will have to pay all your out-of-pocket costs.

      Applying for coverage

      You can change or apply for health care coverage during open enrollment which is November 15, 2014 through February 15, 2015.

      Outside of the open enrollment period, you can apply or change coverage only if you experience a triggering event. For example, if you get married, move into a Kaiser Permanente service area, or lose coverage because you lose your job ─ all triggering events ─ you will have a Special Enrollment Period (SEP). An SEP lasts 60 days after the triggering event occurs. This means you have 60 days from the day of the triggering event to change or apply for health care coverage.
