If you enrolled in a Kaiser Permanente health plan on the Covered California website — also known as the state’s Health Insurance Marketplace — you may view, download, and print a PDF copy of that plan’s 2014 Membership Agreement below. This document is also called the Evidence of Coverage (EOC).
The Membership Agreement/EOC explains your benefits in detail as well as how to get care, and contains contractual information such as your rights and responsibilities as a member.

Your plan’s official name is in the letter welcoming you to Kaiser Permanente. If you have not received this letter yet, the name of the plan as it appears on Covered California is shown in parentheses.
