What you would want to happen if you suddenly became seriously ill? By completing a legal form called an advance health care directive, you can make sure your wishes are respected if you become too ill to make your own health decisions. This form is the only way to legally ensure that your wishes will be met — and it’s important to complete at any age.
We offer a free two-hour class for members and guests who want more information. You’ll receive advance health care directive forms to take home and, in consultation with your families, fill out as part of your permanent health record.
“It’s best to explore your goals, values, and thoughts about quality of life when you’re healthy,” says Margot Fermer, RN, manager of Palliative Care at South Sacramento. “Don’t wait until your life and health are in crisis and the people around you are emotional.”
