There’s an important member of the health care team at our Santa Rosa Medical Center — you. Physicians and staff are collaborating with members and their families to implement a model known as patient- and family-centered care.
This approach to health care recognizes the vital role that families play in supporting patients’ health and well-being. Core principles that you will find at all our facilities include:
Putting those principles into action can be as simple as having doctors discuss treatment plans at a patient’s bedside instead of in the hall, says Jean Ingram, program manager for patient- and family-centered care.
“If the doctors do their rounds in front of you, you gain a lot of information. You and your family may also have important insights to share,” explains Ingram.
At facilities where patient and family-centered care is practiced, studies show that patients do better and feel more satisfied with their care. Studies also show it helps reduce errors and helps to use hospital resources more efficiently.
At our Santa Rosa Medical Center, the new care model effort is being led by an advisory council of members and medical center leaders, which meets monthly to discuss every aspect of care.
