My Medical Records

Current KP members can access most of their medical records at any time from kp.org. Access your record by doing the following:

  • Log in and choose Medical Record from the menu at the top.
  • Select from options on the menu on the left side of the screen and follow the onscreen prompts to view and/or download the most commonly requested medical records, including immunizations, test results, visit summaries, and hospital stays.

Current KP members, former members, and third parties can request specific medical records to be sent to them or another party in electronic or printed form by doing the following:

— Complete and sign in ink the Authorization for Kaiser Permanente to Use/Disclose Protected Health Information.

  • If the authorization is not signed, the requested information will be sent to you, the patient.
  • Initial the appropriate areas of the form if specially protected health information is to be released (HIV, drug and alcohol, mental health or genetics).

— Send an email or mail your request to the Release of Information department including what portion of your medical record you are requesting and where to send it, whether that is to you or another party (for other parties include the name of entity, complete address, phone number, and email address)
— Include payment if the requestor is not the patient (see Cost of Records below).
— Send the completed, signed form and payment (if required) via email, or postal mail to:

Cost of records
There is no cost to current or former members requesting their own medical records. Third parties are charged a flat fee of $16.50 for an electronic release or $16.50 plus postage if paper records are requested. Include local sales tax if records are to be sent to Washington state. There may be additional charges for radiology images or medical records more than 20 years old.

There is no charge for transferring information to non-Kaiser clinicians for ongoing medical care. Please complete the steps above and include the clinician’s complete mailing address, and phone number.
