We have three locations to serve you. Please contact us in any of the ways listed below.

How may we help you?

Our department staff will coordinate the release of your health information. Please call us for specific instructions about your request.

You may call us from 8 am to 4:30 pm, Monday through Friday. In Portland, call 503-571-5051. All other areas, call 1-800-813-2000 and request extension 31-5051.

If you need your X-ray films, please contact our Radiology Department at 503-571-8451.

You may drop off a release of information request at any Membership Services desk. Membership Services representatives will forward all requests to our department via interoffice mail. Please allow 48 hours for delivery to our department.

We usually mail the information to the address you give us if we have your authorization form. If you would prefer to pick up your requested records or forms, please call us to make arrangements. We are located on the Kaiser Sunnyside Medical Center campus in the Regional Process Center building.

Release of Information is sometimes incorrectly referred to as Medical Records. The Release of Information department facilitates releasing medical information to outside requesters and completion of forms. These forms include but are not limited to Short Term Disability, Long Term Disability, and FMLA.
